Consent to Receive Electronic Communication

Many school districts have sent home the Consent to Receive Electronic Communication and plan to upload into associated files. To get the information loaded onto the A page, a Consent to Receive Electronic Communication needs to be loaded into the online forms documents. Once the information is put into the form (date of signature & I give consent/Do not consent checkbox) then it will be populated onto the A page. On the B page in the “Other Essential Information” make a note that the signed consent is uploaded into Associated Files.
